Here at Cygnus Support, we have an exciting opportunity for the right person to join our existing team of Community Link Social Prescribers, to help make a difference to patients’ well-being across the Wansbeck Primary Care Network. This is a ‘social prescribing’ role and is part of the innovative and evolving dynamic approach in delivering health and social care to patients in the region.

We are seeking a highly motivated and passionate person to work with patients to empower them to take control of their health and wellbeing.
